Network4Health (N4H) was originally established in 2016 as part of NH’s 1115 Transformation Waiver which was made possible by funding from the Center for Medicare and Medicaid Services.
Originally titled as an ‘IDN’ or ‘Integrated Delivery Network’, Network4Health represented region or IDN 4 which comprised 18 communities surrounding Greater Manchester, Derry and Salem. This region also held more than 25% of the state’s Medicaid population.
There were a total of seven regions in the whole state.
The work of the 1115 waiver focused on a number of projects aimed to increase the health outcomes of the Medicaid population.
Working in collaboration with our partners, experts can assess an agency’s program structure and services aimed at treating individuals who experience both a mental health and substance use issues. The Co-Occuring Disorder Program aims to increase the knowledge, skills and resources available to partner agencies in regards to identifying and treating the co-occurring disordered population.
Our team is working to develop the HIT infrastructure required to support high quality, integrated care throughout our region and state.
Our enhanced delivery system effectively and efficiently prevents, treats, and manages acute and chronic behavioral health and physical illnesses across multiple providers and sites of service.
Network4Health helps support patients with serious mental illness and/or substance misuse disorders as they transition from different life changes using the Critical Time Intervention (CTI) Model.
The Partial Hospitalization Program (PHP) is designed to help increase the region's capacity of Behavioral Health
and Substance Abuse services.
When it comes to increasing the behavioral health workforce, we at Network4Health are not only thinking short term , but long term as well.
